Procedure for a request deposit

INDIVIDUAL MEASURES OF THE OPPORTUNITIES FUND FOR PEOPLE WITH DISABILITIES (OFPD)

1. ELIGIBILITY

  • The participant has significant and persistent disabilities;
  • The participant is unemployed at the time of the request;
  • The participant is not eligible for Employment insurance: to be sure, ask for a positive response with the request form for verification of eligibility.

Verification of eligibility:
Send the form by fax to Linda Galiazzo at 418-522-7042.
Eligibility is valid for 2 months.

Note: Communicate with your project officer for all unusual situations.

2. REQUESTS PROCESSING

 

Send the demand to your project officer :
  • Information on the participant;
  • Plan of intervention and application for subsidy;
  • Copy of the verification of eligibility response;
  • For Self-employed participant, business plan approved or in planning with the collaboration of a known organization (CLD, SADC, and FCE).

All documents must be signed by sponsor, participant and service provider.

3. PROJECTS APPROVAL

The participant is not allowed to start the project before the project officer’s decision as the participant must be unemployed for eligibility.

4. FOLLOW-UPS
  • Follow-ups are to be made every 3 months;
  • Follow-ups will be planned at the start of the agreement;
  • Complete the last page of the Plan of intervention and application for subsidy according to the objectives of the project and aimed results;
  • Follow-ups can be sent by fax, mail, or e-mail or can be done by phone.

Salaries and Contributions Reimbursement of the participant’s wages and the employment related costs (up to 115%).
Living expenses Income support allocations paid to self-employed workers during business pre start-up or start-up.
Support measures  - adapted equipment Required equipment (except fixed assets) to adapt a work station. Examples: specialized softwares, ergonomic chair.
Support measures- accompaniment Compensation of the person who coaches or supervises the participant.
Support measures- others Fees for interpretation, alternate formats, daycare, accommodations, meal expenses, security material to enter the workforce.
Professional fees Fees for an occupational therapist or any other specialized consultant.
Fixed assets for adaptation Required specialized equipment or facilities that must be developed to make the workplace accessible.
Transportation Transportation fees.
  1. Salary subsidies : interim financial support that can facilitate the participant’s short-term integration into sustainable employment.
  2. Work experience : interim financial support that can allow participants to improve their employability by developing good work habits and by increasing self confidence through the acquisition of transferable work experience.
  3. Assistance for self-employed workers : financial support allowing participants to start up a small business and thereby create his or her own job.
  4. Enhanced employment assistance : internship that may or may not be remunerated and that may enable the participant to acquire the necessary skills to hold a job.
